Expression of Interest for Walk with a Doc

Please fill this out if you are interested in either leading or participating in free Walk with a Doc (or Just Walk) events. 

NB: Locally-based GPs will lead “Walk with a Doc” events, and non-doctorate healthcare professionals will lead ”Just Walk” events. 

Council is collecting your personal information (e.g. name, email, phone number, etc.), for the purposes of registering your interest in participating in the Eat, Breathe, Move Program as outlined on this form. This information will be accessed by authorised Council officers and disclosed to Council’s contracted suppliers under the Eat, Breathe, Move Program.
