This survey is for the students that attended the recent RYPEN camp from Friday 22-24 April in Yeppoon.
The questions will ask about your experience. This information will assist us in understanding if the camp has helped or made a change in your thinking on leadership and personal growth.
All responses are anonymous and we ask you to create an 8 digit number, using the first 4 digits of your birth date (day and month) and the last 4 digits of your mobile number:
Eg: birthday 2 May 2002 – 02/05/2002 and mobile number 0406 952 687 = 0205 2687
If you have any questions please do not hesitate in contacting Andrea Bax: Community Resilience Coordinator: andrea.bax@bundaberg.qld.gov.au or 4130 4175

Thank you for taking the time to complete this survey.
The survey will only take about 15- 20 minutes. There are no right or wrong answers. We would like to hear your honest opinion. This survey will build on the survey that you completed at the start of the Community Resilience Program.
The following questions will ask you about your attitudes and behaviours towards volunteering, your knowledge on natural disaster preparation, response, and recovery and the training that has been delivered to you. It will also ask you about how well you think you cope with challenges, how you are feeling and your plans for the future.
To understand how things change over time we have been tracking your responses across the different surveys you have completed using your unique Participant ID number. Your participant ID number is your birthday (day and month only) and the last four numbers of your phone number. For example, if your birthday is fifth of April and your phone number is 04 1234 4321 your participant number will be 0504 4321

Thank you for taking the time to complete this survey.
The survey will only take about 5-10 minutes. We would like to hear your honest opinion.
The following questions will ask you about your child's involvement in the Community Resilience Youth Program.
The aim of our program has been to work with young people directly to understand their attitudes and behaviour's towards volunteering, knowledge on natural disaster preparation, response, and recovery and what they think about their community. To understand this we have had students do a survey at the start and end of the program.
We would also like to know what parents have thought about the whole process now that we are at the end. All information you provide is confidential, and will only be used to help us look at the effectiveness of the project.
We also need to thank you for your continued support and patience during the program and all the running around that parents need to do for projects like this to happen, it is greatly appreciated.
